Recognize
The signs on the monitor, the image and the patient.
- LabCreatinine up 0.3 mg/dL within 48 hours or 1.5 times baseline within 7 days (KDIGO AKI) 1.
- LabOlder contrast studies used a rise over 25 percent or over 0.5 mg/dL 2.
- LabCreatinine starts rising at 12 to 24 hours, peaks at 48 to 96 hours, and usually returns to baseline by days 3 to 5, sometimes 7 to 10 2.
- PatientUsually silent; oliguria, fluid overload or hyperkalemia when severe.
- LabHighest risk: eGFR under 30, heart failure, shock, diabetes, anemia, age over 75, an IABP and large contrast volumes 32.
Act now
In the order the operator does them. The first three come first, whatever else is happening.
- Check creatinine at 48 to 72 hours after the case in high risk patients 23.
- Volume: hydrate with normal saline unless the patient is overloaded 3.
- Stop nephrotoxic drugs such as NSAIDs where possible 3.
- Avoid another contrast load until the creatinine falls, unless the benefit is clear.
- No specific treatment once established; dialysis for the usual indications 3.
- Nephrology input for a large rise, oliguria, hyperkalemia or overload.
Equipment
From the complications cart. Linked items open in the equipment directory.
- Contrast saving toolsContrast management systems and an injector with a small injection volume 3. Directory: Miser contrast management system, ACIST CVi contrast delivery system.
- Iso osmolar contrastAssociated with lower risk in Brilakis 4. Directory: Visipaque (iodixanol).
- IVUS and physiology for zero or low contrast PCIDirectory: Eagle Eye Platinum, PressureWire X.
Drugs
Adult doses as the cited source gives them. Check your formulary and the label.
Who to call
The escalation ladder, first call first.
- Nephrology: for oliguria, hyperkalemia, overload or a large rise.
- Pharmacy: to adjust renally cleared drugs.
The bailout step by step
A contrast plan for a patient with advanced kidney disease 35
- Estimate risk (Mehran score or NCDR model) and the eGFR 32.
- Hydrate with normal saline before, during and after, unless contraindicated 35.
- Set a contrast limit: below the creatinine clearance in mL in advanced CKD; a contrast to clearance ratio over 2 predicted injury 3.
- Use 5 Fr catheters without side holes, small injections of 2 to 3 mL, diluted contrast and prior films 3.
- Use IVUS, physiology and wire markers instead of injections 3.
- Stage multivessel PCI.
- Creatinine at 48 to 72 hours 2.
Prevent
After care and documentation
- Record the contrast volume, the ratio to creatinine clearance and the creatinine trend.
- Arrange the creatinine check if the patient goes home the same day.
Practice
No emergency simulator case for this; every simulator in the app counts contrast at the top of the screen. Browse the case library.
